This statement outlines how Sacred Heart Medical Center manages your personal information (DATA) to ensure your privacy, protection, and confidentiality, in compliance with the Implementing Rules and Regulations of Republic Act. No. 10173 or Data Privacy Act of 2012 (“DPA”).
What is PERSONAL INFORMATION(DATA)?
Personal information refers to, but is not limited to, the following information pertinent to your personal identity and relevant to the diagnosis and management of your condition:
- Complete name (First, Middle, Last), birth date, birth place, age, sex, nationality, civil status, religion
- Home address, contact numbers (landline and cellular phone), email address
- Occupation, employer’s name, office address, office contact number
- Spouse’s name (if married), cellular phone number, occupation
- Parents’ / Guardian’s Names (if still alive)
- Medical history – family and personal, including previous hospitalizations
- Responsible party, relation to patient
- Educational background, previous employment (for applicants wishing to be employed by SHMC)
Why do we collect your DATA?
We collect your DATA for the primary purpose of sharing them with our partners in health care, including (but not limited to):your Attending Physicians; our Nursing Services; our diagnostic facilities; your Health Maintenance Organization (HMO, if any); the Philippine Health Care Insurance Corporation (Philhealth); the Philippine Charity Sweepstakes Office (PCSO); the Department of Social Welfare and Development DSWD); the Department of Health (DOH); and various government financial assistance agents who may play a role in providing you the appropriate, effective and efficient delivery of qualityservices needed to address your medical/surgical condition, with your signed consent. We also need your DATA to be able to contact you/communicate with you. You may withdraw your consent for the processing and sharing of your DATA (subject to applicable Privacy Laws) by informing us in writing.
How do we collect your DATA?
We will collect and process your DATA through the following means: interviews, application forms, Admission and Discharge Summaries, results of diagnostic work-ups, Doctors’ Orders, Nurses’ Notes, signed Consents/Waivers, official social media accounts like Facebook/Twitter, and from other sources like your HMO, previous hospital records, previous employment, telephone inquiries, letters received from you, and/or response to hospital surveys.
How do we protect/secure your DATA?
Your DATA are collected and stored for safekeeping by our Management Information Services System (MISS), Medical Records, Office of Human Resources Management, Doctors’ Clinics, concerned Departments/Units which regulate their accessibility, alterations, disclosure and disposal according to policies set by the Department of Health and/or the Department of Labor and Employment (DOLE).
When your DATA are no longer need for the purpose for which they were obtained, we will take precautionary measures to dispose of them, making sure to permanently keep your identity anonymous. Otherwise, your DATA will be digitally stored and hard copies be filed for a maximum of ten (10) years, as mandated by the Department of Health.
When/how/to whom do we disclose your DATA?
With your consent, we may disclose your DATA to authorized individuals and/or third parties through the hospital’s electronic information system, through issuance of certified true copies of your hospital/employment records, through fax or email for whatever applicable or relevant medical/legal purposes they may serve.
How can you have access to your DATA?
You can have access to your DATA, personally, or through your authorized representative (upon issuance of written permission) after presenting a valid identification, prior to release of your requested DATA, according to your stipulated rights as DATA subject to update or correct your DATA, stop the processing/sharing of your DATA, or withdraw your consent to the processing/sharing of your DATA.
